Event: create incident from Form Designer form

Answer: To make sure an email is sent when a form has been filled in, in the Self-Service Portal, do the following: Add 'Default entry' option in drop-down list - Go to Module settings > Call Management > Searchlists - In the 'Default entry' field, enter 'SSP form' - Save and refresh the settings. FIll in default entry in forms - Go to Modules > Form Designer.
